The examples use real domain names (which exist and belong to
someone); they probably should use example.com/net/org, unless there
are strong reasons not to do so.

Section 2.5.2 says "The attribute "T38MaxBitRate" is incorrectly
registered with IANA as "T38maxBitRate" (lower-case "m")" -- Should
this say "was once incorrectly registered"? (current registry seems
to have upper-case "m")
